Clinical Observation of Intraoperative Bile Culture in Biliary Tract Disease


Abstract
Intraoperative bile culture data from 171 cases of biliary tract surgeries performed at the department of surgery of Ewha Womans University Hospital during the period from January 1989 to April 1992 were analyzed to determine culture rate, common
organisms isolated, correlation between GB and CBD bile culture of various biliary tract disease, and to predict infectious complications after biliary tract surgeries.
@ES The results were as follows;
@EN 1) Bile culture was positive in 107 cases(62.5%).
2) The most frequent organism was E.coli and Gram negative organisms were more than 90%,
3) In GB stone only, with cultured organism pattern in GB bile, we could predict the organism pattern in CBD bile.
4) There was few difference between the results of GB and CBD bile culture in coexisting GB and CBD stone.
5) In biliary stone without GB stone, the results of GB bile culture was same as that of CBD.
6) Anaerobic cultures were positive in 14 cases(8.2%).
7) Postoperative infectious wound complications were positive in only 9 cases with some correlation between bile culture and wound culture especially E. coli(33.3%). So, we recommend the use of prophylactic antibiotics before and after surgeries
for
biliary tract disease.
